How to get a medical cannabis card

In 2018, medical cannabis was legalized in Utah for patients with qualifying medical conditions. Patients are required to consult with a Qualified Medical Provider (QMP) authorized by the Utah Department of Health and Human Services to receive a medical cannabis recommendation, after which they will obtain a medical cannabis card. With a valid medical cannabis card, patients can access any licensed medical cannabis pharmacy in Utah. It is important to note that bringing cannabis into Utah from another state is illegal, even with an active medical cannabis card.

If you have a qualifying medical condition and medical records or a statement from your treating provider and would like to meet with one of our QMPs to obtain a card, please call the office at to make an appointment.

State Application Process

Before meeting with a QMP, it is highly recommended to create a Utah ID and complete the electronic application using the state's Electronic Verification System (EVS) website.

Cost

We do not bill insurances for medical cannabis evaluations. We have two pricing structures for medical cannabis evaluations.

All initial appointments require a refundable $125 deposit that may be paid via cash, credit or debit card. The deposit may be applied to the cost of the office visit or directly refunded when you show up to your appointment. If you miss your appointment or are more than 15 minutes late, the deposit is not refundable.

Option 1: Cash pay (no cards or checks)

  • Initial visit: $250
  • Renewals:
    • For 6 month cards: $125​
    • For 1 year cards: $250
      • Note: there are times when we will not allow a 1 yeard card. This depends on how stable your condition is and any other underlying medical/health problems.​

Option 2: Membership Plan (no contract) - If you cancel your membership, you will have to establish care with a new QMP

  • Initial visit: $125 (one time only fee - may use deposit for this setup fee)
  • Monthly subscription fee: $35 (begins on 2nd month) autodrafted from a credit or debit card

State Fee: Paid directly to the state each time a new card is issued or renewed: $15​
